Peer Support Services
Empowering individuals in recovery through shared lived experience and personalized mentorship.
Individual and Transitional Support (1915i)
This service provides structured, scheduled interventions to improve a beneficiary’s ability to manage Instrumental Activities of Daily Living (IADL) and promote independent functioning in the community and recovery.
Outpatient Therapy
Supportive, goal-focused counseling for emotional wellness and sustainable recovery outcomes.
Community Living Support (1915i)
Support providing permanent, ongoing assistance for individuals to live independently in their own homes.
